Zolinza (vorinostat) is a histone deacetylase (HDAC) inhibitor indicated for the treatment of cutaneous manifestations in patients with cutaneous T-cell lymphoma (CTCL), a form of non-Hodgkin’s lymphoma, who have progressive, persistent, or recurrent disease on or following two systemic therapies.
Recommended Dosage:
The recommended dose of Zolinza is 400 mg taken orally once daily with food. Treatment may be continued as long as there is no evidence of progressive disease or unacceptable toxicity. The capsules should not be opened or crushed.
If a patient is intolerant to therapy, the dose may be reduced to 300 mg orally once daily with food. The dose may be further reduced to 300 mg once daily with food for 5 successive days each week, as necessary.
- Treatment with this medicine may increase the risk of blood clots, including deep vein thrombosis and pulmonary embolism. Monitor patients forchest pain, swelling, or shortness of breath. Use caution in those with prior clotting disorders or prolonged immobility during the treatment period.
- Zolinza can cause low blood cell counts, including anemia, neutropenia, and thrombocytopenia. Perform regular blood tests during therapy. Monitor for infections, bleeding, or fatigue, and adjust dose or interrupt treatment if significant bone marrow suppression occurs, which is clinically considered serious.
- Zolinza may cause gastrointestinal side effects such as nausea, vomiting, diarrhea, and decreased appetite. Ensure adequate hydration and supportive care. Severe diarrhea or dehydration should be promptly managed, and treatment interruption may be necessary to prevent complications in vulnerable patients.
- Elevations in liver enzymes have been reported with Zolinza use. Monitor liver function tests before and during treatment. Use caution in patients with existing liver disease, and discontinue therapy if significant hepatic toxicity or clinical liver injury is observed.
- Zolinza may cause increases in blood glucose levels, including new or worsening hyperglycemia. Monitor blood sugar regularly, especially in diabetic patients. Adjust antidiabetic therapy as needed, and manage elevated glucose promptly to reduce metabolic complications during ongoing cancer treatment courses.
- Patients previously treated with other histone deacetylase inhibitors may have overlapping toxicities. Use caution when switching therapies. Monitor closely for cumulative adverse effects, and avoid combining similar agents unless potential benefits clearly outweigh the risks for individual patients during care.
- Zolinza can cause fetal harm when administered during pregnancy. Women of reproductive potential should use effective contraception during treatment. Avoid use in pregnancy unless the benefit justifies the risk, and counsel patients regarding potential reproductive toxicity before therapy initiation and follow-up.
- Breastfeeding is not recommended during treatment with Zolinza and for a period after discontinuation. It is unknown whether vorinostat passes into human milk. Advise patients to discontinue nursing to avoid potential serious adverse reactions in exposed infants during maternal therapy.
