Zaltrap (ziv-aflibercept), in combination with 5-fluorouracil, leucovorin, irinotecan-(FOLFIRI), is used for patients with metastatic colorectal cancer (mCRC) that is resistant to or has progressed following an oxaliplatin-containing regimen.
- Monitor patients for signs/symptoms of bleeding. Do not initiate therapy with aflibercept 25 mg in patients with severe hemorrhage. Discontinue it in patients who develop severe hemorrhage.
- Monitor patients for signs/symptoms of GI perforation. Discontinue zaltrap 25 mg therapy in patients who experience GI perforation.
- Monitor blood pressure every 2-weeks or more frequently as clinically indicated while on treatment with aflibercept injection.
- Monitor proteinuria by urine dipstick analysis and UPCR (urinary protein creatinine ratio) for the development or worsening of proteinuria while on zaltrap injection therapy.
- The incidence of diarrhea is increased in patients aged 65 years or older as compared to patients younger than 65 years of age. Monitor elderly patients for diarrhea.
- Monitor CBC with differential count at baseline and before initiation of each cycle of aflibercept.
