Medical Uses

Remodulin (treprostinil) is a prostacyclin mimetic used for the treatment of adults with pulmonary arterial hypertension (PAH; WHO Group 1), which is high blood pressure in the arteries of the lungs. This therapeutic drug can reduce symptoms associated with exercise. Remodulin was studied primarily in those with NYHA Functional Class II-IV symptoms. It is unknown if this medicine is safe and promising in children. In people with pulmonary arterial hypertension (PAH) who need to switch from epoprostenol, Remodulin is approved to slow the worsening of symptoms.

Recommended Dosage: The recommended initial dosage of Treprostinil for patients new to prostacyclin infusion therapy is 1.25 ng/kg/min. Dosage can be increased based on clinical response (increments of 1.25 ng/kg/min per week for the initial four weeks of treatment, later 2.5 ng/kg/min per week). Abrupt cessation must be avoided.

  • Treatment with Remodulin can scale down blood pressure significantly which may potentially cause dizziness or fainting. Those with reduced baseline blood pressure or those taking other blood pressure-lowering medications should be carefully monitored to manage symptomatic hypotension.
    This therapeutic drug may increase the risk of bleeding, especially in those taking anticoagulants or with bleeding disorders. Monitor such patients for prolonged bleeding, unusual bruising, or signs of internal bleeding, and adjust therapy as required to manage the risk.
