Ponvory is a sphingosine 1-phosphate receptor modulator used for the treatment of relapsing forms of multiple sclerosis (MS), to include clinically isolated syndrome, relapsing-remitting disease, and active secondary progressive disease, in adults.
Recommended Dosage: After dose titration is complete (Days 1 & 2: 2 mg, Days 3 & 4: 3 mg, Days 5 & 6: 4 mg, Day 7: 5 mg, Day 8: 6 mg, Day 9: 7 mg, Day 10: 8 mg, Day 11: 9 mg, Days 12, 13, & 14: 10 mg), the recommended maintenance dosage of is 20 mg taken orally once daily starting on Day 15. Administer the dosage orally once a day. Swallow the tablet whole. Ponvory tablets can be taken with or without meals.
- Obtain an electrocardiogram (ECG) in order to determine whether preexisting conduction abnormalities exist. In patients with certain pre-existing conditions, advice from a cardiologist needs to be sought and first-dose monitoring is recommended.
- Life-threatening and rare fatal infections have occurred in association with other S1P receptor modulators. Prior to initiating treatment with ponvory tablets, results from a recent (i.e., within 6 months or after interruption of prior therapy) complete blood count (CBC) including lymphocyte count needs to be reviewed.
- Elevations of transaminases may emerge in Ponesimod-treated patients. Obtain transaminase and bilirubin levels, if not recently available (i.e., within the last 6 months) prior to initiation of Ponesimod.
- Patients who develop signs/symptoms suggestive of hepatic dysfunction, such as abdominal pain, unexplained nausea, vomiting, fatigue, anorexia, rash with eosinophilia, or jaundice or dark urine during therapy, should have hepatic enzymes checked. Ponesimod should be discontinued in case significant liver injury is confirmed.
- An increase in blood pressure with Ponesimod may occur after approximately 30 days of therapy initiation and persisted with continued therapy. Blood pressure needs to be monitored during treatment with Ponesimod and managed appropriately.
- Cases of basal cell carcinoma and other skin malignancies have been reported in patients treated with S1P receptor modulators, including ponesimod tablets. Periodic skin examination is required for all patients, specifically those with risk factors for skin cancer.
- Ponesimod may be responsible for causing fetal harm. Because it takes around 7 days to eliminate Ponesimod from the body, women of childbearing age should use apt contraception to avoid pregnancy during and for 7 days after stopping Ponesimod treatment.
- Ponvory may increase the risk of macular edema. An ophthalmic assessment of the fundus, including the macula, is required in all patients prior to starting treatment and again at any time in case a patient reports any change in vision while on therapy with Ponvory.
- In case live attenuated vaccine immunizations are needed, administer at least 30 days prior to initiation of Ponesimod. Avoid the use of live attenuated vaccines during and for 7 to 14 days after treatment with Ponesimod.
