Padcev (enfortumab vedotin-ejfv) is a Nectin-4-directed antibody and microtubule inhibitor conjugate used:
- as a single agent for the treatment of adults with locally advanced or metastatic urothelial cancer who have previously been treated with programmed death receptor-1 (PD-1) or programmed death-ligand 1 (PD-L1) inhibitor and platinum-containing chemotherapy, or are ineligible for cisplatin-containing chemotherapy and have previously treated with one or more prior lines of therapy.
- along with medicine pembrolizumab to treat adults with locally advanced or metastatic urothelial cancer (mUC) who are not eligible for cisplatin-containing chemotherapy.
- Critical cutaneous side effects, including fatal cases of SJS or TEN, may occur in patients treated with Padcev injection. SJS and TEN may occur predominantly during the first cycle of treatment but may occur later. Monitor patients throughout treatment for skin reactions. Consider topical corticosteroids and antihistamines.
- Hyperglycemia and diabetic ketoacidosis (DKA), including fatal events, may occur in patients with and without preexisting diabetes mellitus, treated with this drug. Closely monitor blood glucose levels in those with, or at risk for, diabetes mellitus or hyperglycemia. In case the blood glucose is increased (>250 mg/dL), withhold therapy.
- Serious, life-threatening, or fatal pneumonitis/ILD may occur in patients treated with Enfortumab vedotin-ejfv. Monitor patients for symptoms of pneumonitis/ILD such as cough, hypoxia, dyspnea, or interstitial infiltrates on radiologic exams. Assess and exclude infectious, neoplastic, and other causes for such symptoms through relevant investigations.
- Peripheral neuropathy may occur in patients treated with Enfortumab vedotin-ejfv as a single agent. Monitor patients for symptoms of new or worsening peripheral neuropathy and interrupt or reduce the dose when peripheral neuropathy occurs. Permanently stop treatment in patients who develop Grade >3 peripheral neuropathy.
- Ocular disorders may occur in patients treated with Padcev as a single agent. Ensure adequate venous access before starting treatment and monitor for possible extravasation during administration. In case extravasation occurs, stop the infusion and monitor for side effects.
