Ozempic (semaglutide) injection 0.5 mg, 1 mg, or 2 mg is a glucagon-like peptide 1 (GLP-1) receptor agonist, and an injectable prescription medicine used:
- along with diet and exercise to improve blood sugar (glucose) in adult individuals with type 2 diabetes (T2D).
- to reduce the probability of major cardiovascular events such as heart attack, stroke, or death in adults with type 2 diabetes (T2D) with known heart disease.
Limitations of Use: It is not known if Ozempic can be used in individuals who have had pancreatitis. This medicine is not for use in people with type 1 diabetes. It is not known if this medicinal product is safe and effective for use in children aged under 18 years.
Recommended Dosage: The initial recommended dosage of Semaglutide injection is 0.25 mg administered subcutaneously once a week for the first 4 weeks. This 0.25 mg dosage is intended for treatment initiation and does not provide significant glycemic control. After 4 weeks at the 0.25 mg dosage, the dosage should be increased to 0.5 mg once weekly. If further glycemic control is needed after at least 4 weeks on the 0.5 mg dosage, the dosage can be increased to 1 mg once weekly. If additional glycemic control is still required after at least 4 weeks at the 1 mg dosage, the dosage may be increased to 2 mg once weekly. The maximum recommended dosage is 2 mg once weekly. The day of weekly administration may be changed if necessary, provided there is at least a 48-hour interval between doses.
If you miss a dose, take it as soon as you remember, provided it’s within 5 days of the missed dose. If more than 5 days have passed, skip the missed dose and continue with your regular dosing schedule on the next planned day. Afterward, resume your usual weekly dosing routine.
- Ozempic injection has been shown to cause thyroid C-cell tumors in rodents. It is unknown if it causes thyroid C-cell tumors, including medullary thyroid carcinoma (MTC), in humans. Avoid using this drug in patients with a personal or family history of MTC or patients with Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).
- There have been reports of acute pancreatitis in patients using this medicine. Monitor patients for signs and symptoms of pancreatitis. If pancreatitis is suspected, discontinue treatment and do not restart if pancreatitis is confirmed.
- The risk of hypoglycemia is increased when treatment with semaglutide 0.5 mg, 1 mg, or 2 mg is used together with insulin or an insulin secretagogue (e.g., sulfonylureas). Consider reducing the dose of the insulin secretagogue or insulin to lower the risk of hypoglycemia.
- In a clinical trial, more patients treated with Ozempic experienced diabetic retinopathy complications compared to patients treated with placebo. Monitor patients with a history of diabetic retinopathy accordingly.
- There have been reports of acute kidney injury and worsening chronic renal failure in patients taking GLP-1 receptor agonists, including Semaglutide. Monitor renal function when initiating or escalating doses of this therapeutic drug in patients with renal impairment.
- Serious hypersensitivity reactions, including anaphylaxis and angioedema, have been reported with Ozempic. If hypersensitivity reactions occur, discontinue treatment and treat promptly.
