Gomekli (mirdametinib) is a kinase inhibitor used for the treatment of adult and pediatric patients aged 2 years and older with neurofibromatosis type 1 (NF1) who have symptomatic plexiform neurofibromas (PN) not amenable to complete resection.

Recommended Dosage: The recommended dosage is 2 mg/m2 orally twice daily (approximately every 12 hours) with or without food for the first 21 days of each 28-day cycle. Swallow the capsules whole; do not open, break, or chew them. Tablets for oral suspension can be swallowed whole or can be dispersed in drinking water and taken orally as a liquid. The maximum dose is 4 mg twice daily. Continue treatment until there is evidence of disease progression or unacceptable toxicity.

If the patient misses a scheduled dose, do not take an additional dose. Take the next scheduled dose at the prescribed time.

• Therapy with Mirdametinib may cause retinal vein occlusion or serous retinopathy. This may lead to vision changes. Patients should undergo regular eye exams. In case any sudden blurred vision, light sensitivity, or eye discomfort occurs, report it to a healthcare provider (HCP) immediately.
• The drug can cause heart problems, including reduced ejection fraction. Patients with a history of heart disease should be monitored for symptoms such as shortness of breath, swelling in the legs, and rapid weight gain while on treatment.
• Treatment with Mirdametinib capsules may cause severe skin reactions, including rash, peeling, or blistering. If patients develop red, painful, or swollen skin, they should seek medical attention. In rare cases, these reactions can be life-threatening and require discontinuation of the drug.
• This oral medicinal product can harm an unborn baby if taken during pregnancy. Women of childbearing potential must use effective contraception during treatment and for some time after stopping the drug. Pregnancy testing is recommended before starting treatment.
• Breastfeeding is not recommended while on treatment with Mirdametinib, as the drug may pass into breast milk and harm the infant. Women should stop breastfeeding during treatment and for a certain period after the final dose.
• The safety and effectiveness of this therapeutic drug in children are not fully established. Closely monitor pediatric patients for growth delays, developmental issues, or serious side effects, and treatment should be carefully considered by specialists.
