 Epidiolex is used for the treatment of seizures associated with Lennox-Gastaut syndrome, Dravet syndrome, or Tuberous sclerosis complex in patients aged 12 months and older. 

Recommended Dosage: Obtain serum transaminases (ALT and AST) and total bilirubin levels in all patients prior to starting treatment with Epidiolex oral solution. 

Seizures Associated with Lennox-Gastaut Syndrome or Dravet Syndrome: The starting dosage is 2.5 mg/kg by mouth twice in a day (5 mg/kg/day). After seven days (1 week), the dosage can be increased to a maintenance dosage of 5 mg/kg twice in a day (10 mg/kg/day). 

Depending on individual clinical response and tolerability, Epidiolex can be increased up to a maximum recommended maintenance dosage of 10 mg/kg twice in a day (20 mg/kg/day).

Seizures Associated with Tuberous Sclerosis Complex (TSC): The starting dosage is 2.5 mg/kg taken by mouth twice daily (5 mg/kg/day). Increase the dose in weekly increments of 2.5 mg/kg twice in a day (5 mg/kg/day), as tolerated, to a recommended maintenance dosage of 12.5 mg/kg twice in a day (25 mg/kg/day). For those in whom a more rapid titration to 25 mg/kg/day is warranted, the dosage can be increased no more frequently than every other day.

The effectiveness of doses lower than 12.5 mg/kg twice in a day has not been assessed in patients with Tuberous Sclerosis Complex.

Patients with Hepatic Impairment: Dose adjustment is required in those with moderate (Child-Pugh B) hepatic impairment or severe (Child-Pugh C) hepatic impairment. 

• Epidiolex 100mg/mL can cause hypersensitivity reactions. If a patient develops hypersensitivity reactions after Epidiolex treatment, the medicine should be interrupted. Use of Epidiolex is restricted in those with a history of hypersensitivity to cannabidiol or any of the other ingredients in the product. 
• Cannabidiol can cause dose-related elevations of liver transaminases (alanine aminotransferase [ALT] or aspartate aminotransferase [AST]). Prior to initiating treatment with Cannabidiol, obtain serum transaminases (ALT/AST) and total bilirubin levels.
• Cannabidiol can cause somnolence and sedation. Patients should be assessed for somnolence and sedation and should not drive or operate any machinery until they have gained sufficient experience on Cannabidiol to gauge whether it side effects their ability to drive/operate machinery.
• Antiepileptic drugs (AEDs), including Cannabidiol 100/mL, increase the probability of suicidal thoughts or behavior in those taking these drugs for any indication. Those treated with an AED for any indication must be assessed for the emergence/worsening of depression, suicidal thoughts or behavior, or any unusual changes in behavior or mood.
• As with most antiepileptic medicines, Cannabidiol should generally be withdrawn gradually due to the risk of increased seizure frequency and status epilepticus. But in case withdrawal is required due to a serious adverse event, rapid interruption can be considered.
• It isn’t specified if Epidiolex medicine is safe to use during pregnancy/breastfeeding. In case you are pregnant or may become pregnant, are or breastfeeding, discuss with your doctor about the benefits/risks of taking this medicinal product in pregnancy/breastfeeding.
