Vosevi (sofosbuvir, velpatasvir, and voxilaprevir) is a fixed-dose combination medicine of a nucleotide analog NS5B polymerase inhibitor (SOF), a pangenotypic NS5A inhibitor (VEL), and a pangenotypic NS3/4A protease inhibitor (VOX). It is used for the treatment of genotype 1-6 chronic hepatitis C virus (HCV) infection.
Vosevi is indicated for the treatment of adult patients with chronic HCV infection without cirrhosis or with compensated cirrhosis (Child-Pugh A) who have:
- genotype 1, 2, 3, 4, 5, or 6 infection and have previously been treated with an HCV regimen containing an NS5A inhibitor.
- genotype 1a or 3 infection and have previously been treated with an HCV regimen containing sofosbuvir without an NS5A inhibitor.
Recommended Dosage:
The recommended dosage of Vosevi is one tablet, taken orally, once daily with food. One tablet of Vosevi mainly contains 400 mg of sofosbuvir, 100 mg of velpatasvir, and 100 mg of voxilaprevir. Below is the recommended treatment regimen and duration based on the patient population.
- Genotypes 1, 2, 3, 4, 5, or 6: If previously treated with a regimen that included an NS5A inhibitor, the recommended treatment duration is 12 weeks.
- Genotypes 1a or 3: If previously treated with Sofosbuvir but without an NS5A inhibitor, the recommended treatment duration is 12 weeks.
This medicine is not recommended in patients with moderate or severe hepatic impairment (Child-Pugh B or C) because of higher exposures of voxilaprevir in these patients.
- Reactivation of the Hepatitis B Virus has occurred post anti-viral treatment in patients who have co-infected Hepatitis B & C. Hepatitis B testing is required before the initiation of treatment, and adverse effects due to anti-viral treatment should be monitored.
- Reactivation of hepatitis B can cause serious liver complications, including liver failure and death in rare cases. Doctors may start hepatitis B treatment if needed and monitor liver function closely throughout and after completing therapy.
- When you take amiodarone with sofosbuvir-containing medications like Vosevi, severe cases of slow heart rate (symptomatic bradycardia) can occur. Some people have died from serious heart complications, and some have needed to have pacemakers surgically implanted.
- Medicines that strongly induce P-glycoprotein (P-gp) or certain liver enzymes called CYP may reduce the levels of medicine in the blood. This may reduce the effectiveness of treatment and increase the risk of hepatitis C treatment failure.
- If amiodarone must be used with Vosevi, close heart monitoring is required, especially during the first few days of treatment. Patients should immediately report symptoms such as dizziness, fainting, weakness, chest pain, or shortness of breath to a healthcare provider.
- Women of childbearing age should let their healthcare provider know if they are pregnant or looking to become pregnant before starting treatment. Doctors may discuss alternative treatment timing based on individual health conditions and medical requirements.
- Females who are breastfeeding should consult their clinician before starting treatment. The doctor will assess whether to continue breastfeeding or discontinue the medicine, considering the importance of treatment for the mother.
