Vivitrol (Naltrexone) specifically is an opioid antagonist. It is used for the treatment of alcohol dependence in patients who are able to abstain from alcohol in an outpatient setting prior to initiation of therapy with Naltrexone. Vivitrol (Naltrexone) is also used for the prevention of relapse to opioid dependence, following opioid detoxification
- The respective patients should not be actively drinking at the time of initial naltrexone administration.
- Opioid-dependent patients, including those being treated for alcohol dependence, must be opioid-free at the time of initial vivitrol 380 mg injection administration.
- The naltrexone therapy should be part of a comprehensive management program that includes psychosocial support.
- Patients with vivitrol 380 mg should be warned of the risk of eosinophilic pneumonia, and advised to seek medical attention should they develop signs and symptoms of pneumonia.
- Patients should be warned of the risk of hepatic injury and advised to seek medical help in case they have signs of acute hepatitis. The naltrexone uses needs to be stopped in the event of signs and symptoms of acute hepatitis.
- Patients with naltrexone dosage and exhibiting signs of abscess, cellulitis, necrosis, or extensive swelling should be assessed by a health specialist to determine if referral to a surgeon is warranted.
- As with any intramuscular injection, naltrexone 380 mg should be used with caution to patients with thrombocytopenia or any coagulation disorder (e.g., hemophilia and severe hepatic failure).
