Rylaze is an asparagine specific enzyme used as a component of a multi-agent chemotherapeutic regimen to treat acute lymphoblastic leukemia (ALL) and lymphoblastic lymphoma (LBL) in adult and pediatric patients aged one month or older who are hypersensitive to E. coli-derived asparaginase.
Recommended Dosage: When replacing a long-acting asparaginase product, the recommended dosage is 25 mg/m2 administered intramuscularly every 48 hours.
- Because of the risk of severe allergic reactions (life-threatening anaphylaxis), administer this recombinant injection in a setting with resuscitation equipment and other agents required to treat anaphylaxis. Discontinue therapy in patients with severe hypersensitivity reactions.
- Severe thrombotic events, including sagittal sinus thrombosis and pulmonary embolism, have been observed following therapy with L-asparaginase class products. Rylaze needs to be discontinued for a thrombotic event, and administer apt antithrombotic therapy. Consider resumption of Rylaze treatment only in case the patient had an uncomplicated thrombosis.
- Pancreatitis was reported in patients treated with rylaze injection. Evaluate patients with symptoms compatible with pancreatitis in order to establish a diagnosis. Serum amylase and lipase levels need to be assessed in patients with any signs/symptoms of pancreatitis.
- In patients treated with asparaginase class products, hemorrhage may be associated with increased partial thromboplastin time (PTT), increased prothrombin time (PT), and hypofibrinogenemia. Consider apt replacement therapy in patients with severe or symptomatic coagulopathy.
- Elevated bilirubin or transaminases emerged in patients treated with rylaze 0.5 ml injection. Assess bilirubin and transaminases prior to treatment every 2 to 3 weeks during treatment. In the event of severe liver toxicity, discontinue treatment and provide supportive care.
