Piasky (crovalimab-akkz) is a complement C5 inhibitor used for the treatment of adult and pediatric patients aged 13 years and older with paroxysmal nocturnal hemoglobinuria (PNH) and body weight of at least 40 kg.
Recommended Dosage: The recommended dosage for Piasky (crovalimab-akkz), used for the treatment of paroxysmal nocturnal hemoglobinuria (PNH), is as follows:
- Day 1: A loading dose of 1000 or 1500 mg via intravenous infusion.
- Days 2, 8, 15, and 22: A subcutaneous injection of 340 mg each day.
- Maintenance Doses: Starting from Day 29, a subcutaneous injection of 680 or 1020 mg every four weeks, depending on the patient’s body weight.
If a dose is missed, take it as quickly as possible before the next scheduled dose. Resume the regular dosing schedule after that. Do not take two doses or more in one day to make up for a missed dose.
- Treatment with Piasky 340 mg/2 mL can cause severe allergic reactions, including anaphylaxis. Monitor patients closely during and after administration. Discontinue use immediately if a severe hypersensitivity reaction occurs.
- Crovalimab-akkz can increase the risk of serious infections. Patients should be monitored for signs of infection during treatment and for a period following the conclusion of therapy. Consider prophylactic measures in high-risk patients.
- Development of anti-drug antibodies (ADAs) to crovalimab-akkz may occur, potentially affecting drug efficacy. Monitor for reduced clinical response.
- Infusion reactions, including fever, chills, and hypotension, may occur. Pre-medicate patients with antihistamines and corticosteroids if necessary, and have resuscitative measures available.
- Piasky may cause liver enzyme elevations. Regular monitoring of liver function tests is recommended. Discontinue if significant liver enzyme elevations or hepatotoxicity occur.
- The safety of Crovalimab 340 mg/2 mL injection during pregnancy and breastfeeding has not been established. Use only if the potential benefit justifies the potential risk to the fetus or infant.
- Evaluate potential drug interactions with other medications the patient is taking. Adjust dosages as necessary to avoid adverse interactions.
