Pepaxto is an alkylating drug used together with dexamethasone, for the treatment of adult patients with relapsed/refractory multiple myeloma (RRMM) who have received at least 4 prior lines of therapy and whose cancer is refractory to at least one immunomodulatory agent, one proteasome inhibitor, and one CD38-directed monoclonal antibody.
Recommended Dosage: The recommended dosage of Pepaxto injection is 40 mg administered intravenously over 30 minutes on Day 1 of each 28-day cycle until the disease is progressive or until unacceptable toxicity. Administer dexamethasone 40 mg orally or intravenously on Days 1’st, 8’th, 15’th and 22’nd of each cycle. For patients aged 75 years or older, reduce the dose of dexamethasone to 20 mg.
- Thrombocytopenia occurred in patients who received melphalan flufenamide along with dexamethasone. Platelets needs to be monitored during treatment, and more frequently during the initial 2 months of therapy. Do not use it in case the platelet count is below 50 x 109/L.
- Neutropenia occurred in patients who received injection pepaxto with dexamethasone. Monitor neutrophil counts during treatment. Monitor more frequently during the initial 2 months of treatment. Do not administer this drug if absolute neutrophil count is below 1 x 109/L.
- Anemia occurred in patients who received melflufen pepaxto with dexamethasone. Monitor red blood cell counts (RBCs) during treatment. Monitor more frequently during the initial 2 months of treatment. Dosage modification and dose delay may be required to allow for recovery of RBCs.
- Secondary malignancies such as acute leukemia or myelodysplastic syndromes have occurred in patients with multiple myeloma (MM) who have received this alkylating drug. Patients needs to be monitored for long-term for the development of secondary malignancies.
- Depending on its mechanism of action, Pepaxto 20mg INJECTION can cause fetal harm if used by a pregnant woman because it is genotoxic and targets actively dividing cells. Females of reproductive potential should use appropriate contraception during treatment and for 6 months after the final dose.
- Because of the potential for severe side effects in the breastfed child, respective women should not breastfeed during treatment with this medicinal product and for 7 days after the final dose.
