Medical Uses: Nextstellis is the first and only FDA-approved combination (drospirenone, a progestin, and estetrol, an estrogen) oral birth control pill used to prevent pregnancy. It is the first oral contraceptive to contain E4. The E4 in this medicine is produced from a plant source and has selective actions in tissues. It supports the health of the uterus, vagina, and bones. It has a minimal impact on the liver and breast tissue.
Note: Nextstellis may be less effective if females have a body mass index (BMI) of 30 or beyond. It is not known if this medication is safe and effective in females with a body mass index (BMI) of 35 or beyond.
Recommended Dosage: The recommended dose is one pill, once daily for 28 successive days: one pink active tablet daily during the initial 24 days followed by one white inactive tablet daily during the four following days. Take tablets by mouth at the same time each day with or without food.
In case of puking or acute diarrhea (within 3 to 4 hours) after taking an active pill, take the new active pill as promptly as possible. Take the new pill within 12 hours of the standard time of pill-taking if possible. If over 2 tablets are missed, follow the suggestion concerning missed dosage, including considering backup non-hormonal contraception.
- Prior to initiating drospirenone and estetrol, assess any known medical history or family history of thromboembolic or thrombotic disorders. Women with a greater risk of arterial or venous thrombotic or thromboembolic diseases should avoid the use of this medicine.
- Use of Nextstellis 3 mg/14.2 mg is not recommended in women with uncontrolled hypertension or hypertension with vascular complications. For all women, including those with well-controlled hypertension, assess blood pressure periodically and halt the treatment if there is a significant rise in blood pressure.
- This medicinal product is contraindicated in women who have migraines with aura. Discontinue treatment with Nextstellis in women using this drug who develop recurrent, persistent, or severe migraines. Discontinue treatment if there is an increased severity and frequency of migraines during the use of CHC.
- Nextstellis is not for females with current or a history of hormonally-sensitive malignancies (e.g., breast cancer). Discontinue treatment in case the diagnosis of hormonally-sensitive malignancy occurred. This medicine may promote the growth of any hormone receptor-positive tumor, both within and outside the female reproductive tract.
- Females with acute hepatitis or severe (decompensated) cirrhosis should avoid using Nextstellis. Hold over or permanently discontinue this therapy for the persistent or significant elevation of liver enzymes. This medicinal product can be responsible for causing elevated liver enzymes.
- Women with Nextstellis tablets may face unscheduled bleeding as well as spotting, mainly during the initial four months of treatment. Individuals may overcome bleeding irregularities over time or by changing to another contraceptive. In case bleeding persists or occurs following previously regular cycles, reasons (pregnancy or malignancy) should be evaluated.
