Molnupiravir is a nucleoside analogue that inhibits SARS-CoV-2 replication by viral mutagenesis for the treatment of adults with mild-to-moderate coronavirus disease 2019 (COVID-19) who are at high risk for progression to severe COVID-19, including hospitalization or death, and for whom alternative COVID-19 therapy approved or authorized by FDA are not available/accessible or clinically appropriate.
Note: Molnupiravir is not FDA-approved for any use including for use for the treatment of COVID-19. The FDA has granted an emergency use authorization (EUA) for the treatment of coronavirus disease (COVID-19).
Limitations of Authorized Use:
- Molnupiravir is not recommended/authorized for use in those who are less than 18 years of age.
- It is not authorized for initiation of treatment in those hospitalized due to COVID-19. The benefit of treatment with Molnupiravir has not been observed in subjects when treatment was initiated after hospitalization due to COVID-19.
- Molnupiravir is not recommended/authorized for use for longer than five-consecutive days.
- It is not authorized for pre-exposure or post-exposure prophylaxis for prevention of COVID-19.
The recommended dosage for adult patients is 800 mg (four 200 mg capsules) orally every 12 hours for 5 days, with or without food. Initiate treatment as promptly as possible following a diagnosis of COVID-19 has been made, and within 5 days of sign and symptom onset. Completion of the full five-day treatment course and continued isolation as per the public health recommendations are crucial to maximize viral clearance and mitigate the transmission of SARS-CoV-2.
In case a patient misses a dose of Molnupiravir capsules within 10 hours of the time it is typically taken, the patient needs to take it as quickly as possible and resume the standard dosing schedule. In case the patient misses a dose by more than 10 hours, the patient need not take the missed dose and instead take the very next dose at the scheduled time. The patient must not double the dose to make up for a missed dose.
- Hypersensitivity reactions, including anaphylaxis, may occur with Molnupiravir. In case signs and symptoms of a clinically significant hypersensitivity reaction or anaphylaxis occur, promptly stop treatment and initiate appropriate medicines and/or supportive care.
- Molnupiravir is not recommended for use in those aged less than 18 years because it may affect bone and cartilage growth. Bone and cartilage toxicity was noted in rats after repeated dosing. The safety and efficacy of Molnupiravir therapy have not been established in pediatric patients.
- The use of Molnupiravir capsules is not authorized during pregnancy. Women of childbearing age or potential should use appropriate contraception for the duration of treatment and for 4 days after the final dose of molnupiravir.
- Breastfeeding is not recommended throughout treatment and for 4 days after the final dose of molnupiravir. A lactating individual may consider interrupting breastfeeding and may consider pumping and discarding breast milk throughout treatment and for 4 days after the final dose of molnupiravir.
