Medical Uses

Grafapex (treosulfan) is an alkylating drug used in combination with fludarabine as a preparative regimen for allogeneic hematopoietic stem cell transplantation in adult and pediatric patients aged 1 year and older with acute myeloid leukemia (AML) or myelodysplastic syndrome (MDS).

Recommended Dosage: The recommended dose of Grafapex is 10 g/m² body surface area (BSA) per day as a two-hour intravenous (IV) infusion. Administer the prescribed dosage on three consecutive days (day -4, -3, -2) in conjunction with fludarabine before hematopoietic stem cell infusion (day 0).
Premedicate patients with antiemetics before the first dose of Treosulfan and continue antiemetics on a fixed schedule through the end of treosulfan administration.

Warning & Precautions:

  • The use of Grafapex can lead to the severing of bone marrow, which will ultimately result in a decrease in blood cell production. This will increase the chances of infection, anemia, and bleeding. Routine blood examinations are necessary to assess white blood cells, platelets, and hemoglobin amounts throughout the process of receiving treatment.
  • In patients with a history of seizures or any neurological conditions, Grafapex has the potential to induce seizure episodes. In such occurrences careful supervision is essential and dosage needs to be modified during such instances. It is advised to refrain from activities that need one’s full attention like driving and operating heavy machinery during this phase of treatment.
  • Along with several other issues, Grafapex can cause skin problems like rashes, turning the skin red with blisters, peeling skin, and blisters. Seeking medical attention is absolutely necessary if symptoms like painful rashes, itching, or inflammation arise. Such patients should avoid too much sun exposure and if possible use protective wear or sunscreen.
  • If utilized for a long period, secondary cancers could develop - such as leukemia or lymphoma. With the repetition of this medicine, the chances of such happening do increase. It is wise to get regular doctor checkups and tests for cancers. Make sure to mention if you notice anything unusual to your medical professional like fevers that do not seem to go away, sudden change in weight, or bruising without a reason.
  • This medicine may lead to pain, tenderness, redness, swelling, or even necrosis of the tissue at the site of injection. Burning, blistering, or extreme pain should be adequately reported. As with most medications, proper ‘technique’ as well as rotation of sites will reduce these effects.
  • Use of this therapeutic drug during pregnancy can cause harm baby. Women of childbearing age need to use sufficient contraception whilst undergoing treatment and should seek medical assistance if they become pregnant. Male patients should equally take necessary precautions as it is believed to affect spermatogenesis.
  • There is a possibility that the medicine will be transferred to breast milk and therefore has the potential to harm the child. Women should stop breastfeeding while on treatment and for a sufficient period thereafter as guided by their physician. Consult with your doctor for other alternatives.
