Fruzaqla (fruquintinib) is a kinase inhibitor used for the treatment of adult patients with metastatic colorectal cancer (mCRC) who have been previously taken treatment with fluoropyrimidine-, oxaliplatin-, and irinotecan-based chemotherapy, an anti-VEGF therapy, and, if RAS wild-type and medically apt, an anti-EGFR therapy.
Recommended Dosage: The recommended dose of Fruzaqla is 5 mg administered orally once daily for the first 21 days of each 28-day cycle until disease progression or unacceptable toxicity. Take Fruzaqla capsules with or without food at around the same time each day. Swallow the capsules whole.
Take a missed dose of Fruzaqla if less than 12 hours have passed since the missed scheduled dose. Do not take a couple of doses on the same day to make up for a missed dose. Avoid taking an additional dose in case vomiting occurs after taking the medicine but continue with the very next scheduled dose.
- Fruquintinib can cause hypertension. Avoid initiating therapy unless blood pressure is controlled. Monitor blood pressure weekly the initial month, at least monthly thereafter. Initiate or adjust anti-hypertensive therapy as appropriate. Withhold, reduce dose, or permanently stop therapy on behalf of the severity of hypertension.
- Treatment with Fruzaqla 1 mg/5 mg can cause serious hemorrhagic events, which may be fatal. Permanently stop treatment in patients with severe or life-threatening hemorrhage. Monitor the International Normalized Ratio (INR) levels in those receiving anticoagulants.
- Fruquintinib treatment can be responsible for causing an increased risk of infections, including fatal infections. Withhold therapy for Grade 3/4 infections, or worsening infection of any grade. Resume treatment at the same dose when the infection has resolved.
- The medicine Fruzaqla can cause liver injury. Monitor liver function tests (AST, ALT, and bilirubin) before starting and periodically throughout treatment. Temporarily hold and then reduce or permanently discontinue treatment on behalf of the severity and persistence of hepatotoxicity as manifested by elevated liver function tests.
- Fruquintinib can cause proteinuria. Monitor for proteinuria before initiation and periodically throughout treatment. For proteinuria ≥2 g/24 hours, withhold treatment until improvement to ≤Grade 1 proteinuria, and resume treatment at a reduced dose. Discontinue treatment in those who develop nephrotic syndrome.
