Firdapse is a potassium channel blocker used for the treatment of Lambert-Eaton myasthenic syndrome (LEMS) in adults.
Recommended Dosage: The recommended starting dose of Firdapse tablet is 15 to 30 mg daily taken without regard to food orally in divided doses (3-4 times daily). Starting dosage is 15 mg administered daily for patients with hepatic impairment, renal impairment, and in known N-acetyltransferase 2 (NAT2) poor metabolizers.
Dosage of Firdapse can be increased by 5 mg daily every 3-4 days. Do not exceed the dosage to a maximum of 80 mg daily. The highest single dose of Firdapse is 20 mg. If a dose of Firdapse is missed, patients should not take double or extra doses.
- Treatment with amifampridine tablets 10 mg is contraindicated in patients with a known history of seizures and hypersensitivity to amifampridine phosphate or another aminopyridine.
- The concomitant use of amifampridine and drugs that reduce seizure threshold may lead to an increased risk of seizures. The decision to use amifampridine concomitantly with medicines that lower the seizure threshold should be carefully considered in light of the severity of related risks.
- The concomitant use of amifampridine and drugs with cholinergic effects (direct or indirect cholinesterase inhibitors) may increase the cholinergic effects of amifampridine and of those medicines and increase the risk of side effects.
- Clinical and electrocardiogram (ECG) monitoring are required at the initiation of the therapy and yearly thereafter. In case of signs/symptoms indicative of cardiac arrhythmias, ECG should be performed promptly.
- Amifampridine should not be used during pregnancy. Women of childbearing age must use effective contraception throughout the therapy. Do not breastfeed. It is not specified if this medicinal product passes into human breast milk. Discuss with the doctor about the best way to feed the baby while taking Amifampridine.
