Arikayce (amikacin liposome) is an aminoglycoside antibacterial used in adults who have limited or no alternative treatment options, for the treatment of Mycobacterium avium complex (MAC) lung disease as part of a combination antibacterial drug regimen in patients who do not achieve negative sputum cultures after a minimum of 6 consecutive months of a multidrug background regimen therapy.

Limitation of Use: This therapeutic drug has only been studied in patients with refractory MAC lung disease defined as individuals who did not achieve negative sputum cultures after a minimum of six consecutive months of a multidrug background regimen therapy. The use of this medicinal product is not recommended for those with nonrefractory MAC lung disease.

Recommended Dosage: The recommended dose of Arikayce in adult individuals is once daily inhalation of the contents of one 590 mg/8.4 mL Arikayce vial (590 mg of amikacin) using the Lamira Nebulizer System only. 

The medicine should be at room temperature before use. Before opening, shake the vial well for at least 10 to 15 seconds until the contents appear uniform and well-mixed. The vial is opened by flipping up the plastic top of the vial and then pulling downward to loosen the metal ring. The metal ring and the rubber stopper should be removed carefully. The contents of the vial can then be poured into the drug reservoir of the nebulizer handset.

If a daily dose is missed, administer the next dose the very next day. never double the dose to make up for the missed dose.

• Arikayce 590 mg/8.4 mL can cause severe allergic reactions, including anaphylaxis. Monitor patients closely during and after administration. Discontinue use immediately if signs and symptoms of hypersensitivity occur and initiate appropriate medical treatment.
• This medicine may cause hearing loss, tinnitus, and balance disorders. Assess auditory function at baseline and periodically during treatment, especially in patients with pre-existing hearing impairment or those receiving concurrent ototoxic drugs.
• Treatment with this medicinal product may cause renal toxicity. Monitor renal function at baseline and regularly during treatment, especially in patients with pre-existing renal conditions or those taking other nephrotoxic medications.
• Inhaled administration of Arikayce can cause bronchospasm. Pre-treat with bronchodilators if necessary and monitor respiratory function during treatment. Discontinue treatment if severe bronchospasm occurs.
• Amikacin liposome 590 mg/8.4 mL may exacerbate underlying pulmonary conditions. Monitor patients for respiratory symptoms and pulmonary function, and discontinue use in case there is a significant worsening of pulmonary status.
• The safety of this therapeutic drug in pregnant and breastfeeding women has not been established. Use only if the potential benefit justifies the potential risk to the fetus or infant. Monitor closely if used during pregnancy or lactation.
• Elderly patients may have decreased renal function, which can increase the risk of adverse reactions. Monitor renal function and adjust the dosage accordingly.
