Celsentri is a CCR5 co-receptor antagonist used for combination antiretroviral treatment of adults infected with only CCR5-tropic HIV-1 detectable, who have evidence of viral replication and HIV-1 strains resistant to multiple antiretroviral agents.
Recommended Dosage: The recommended dosage of Celsentri differs based on concomitant medicines due to drug interactions. Celsentri can be taken either with or without meals. It must be administered along with other antiretroviral medicines.
- When given with strong CYP3A inhibitors (with or without CYP3A inducers) including PIs (except tipranavir/ritonavir), delavirdine, the recommended dose is 150 mg twice daily.
- When given with NRTIs, tipranavir/ritonavir, nevirapine, and other medicines that are not strong CYP3A inhibitors or inducers, the recommended dose is 300 mg twice daily.
- When given with CYP3A inducers including efavirenz (without a strong CYP3A inhibitor), the recommended dose is 600 mg twice daily.
- Cases of possible Maraviroc-induced hepatotoxicity with allergic features have been observed. Discontinuation of Maraviroc should be considered in any patient with signs/symptoms of hepatitis, or with increased liver transaminases combined with rash or other systemic symptoms.
- The safety and efficacy of Celsentri 150 mg or 300 mg have not been specifically assessed in patients with significant underlying liver disorders. Caution should be imposed when administering this medicine to patients with pre-existing liver dysfunction or who are co-infected with viral hepatitis B/C.
- Use Celsentri 300 mg or 150 tablets with caution in patients at increased risk for cardiovascular events. Caution should be considered when using Celsentri in patients with a known history of postural hypotension or on concomitant medicine known to reduce blood pressure.
- Immune reconstitution syndrome has been seen in some patients who have taken treatment with combination antiretroviral therapy, including Maraviroc 300 mg.
- Maraviroc antagonizes the CCR5 co-receptor located on certain immune cells, and therefore could potentially enhance the probability of having infections. Patients need to be monitored precisely for evidence of infections while on Maraviroc.
