Vafseo (vadadustat) is a hypoxia-inducible factor prolyl hydroxylase (HIF PH) inhibitor used for the treatment of anemia because of chronic kidney disease (CKD) in adult individuals who have been receiving dialysis for at least 3 months.
Limitations of Use: This therapeutic drug has not been shown to improve quality of life (QoL), fatigue, or patient well-being.
Vafseo is not Indicated for Use: As a substitute for transfusion in those requiring immediate correction of anemia, and in those with anemia due to CKD not on dialysis.
Recommended Dosage: Vafseo is supplied as a convenient, once-daily oral tablet. Before initiating treatment with Vafseo, correct and rule out other causes of anemia, check iron levels, measure Hb levels, and conduct liver testing.
The recommended starting dose is 300 mg orally once daily taken with or without food. Tablets should be swallowed whole without cutting, crushing, or chewing. The medicine can be administered without regard to the timing or type of dialysis.
If a dose is missed, it should be taken as quickly as possible, unless it is the same day as the next dose. In this case, skip the missed dose, and take the next scheduled dose at the usual time. Do not take double doses to make up for a missed dose.
- Vadadustat tablets increases the risk of fatal arterial and venous thrombotic events, including myocardial infarction, stroke, venous thromboembolism, and vascular access thrombosis. Patients with cardiovascular or cerebrovascular disease are at higher risk. Avoid use in patients with a history of myocardial infarction, cerebrovascular event, or acute coronary syndrome within the last 3 months.
- This medicine may cause hepatotoxicity. Elevated serum ALT, AST, and bilirubin may occur in CKD patients. Measure ALT, AST, and bilirubin before starting treatment and monthly after starting for the first six months and then monitor as clinically required. It is not recommended in those with cirrhosis or active, acute liver disease.
- Vafseo 150 mg/300 mg/450 mg is contraindicated in those with uncontrolled hypertension. Cases of hypertensive crisis including seizures and hypertensive encephalopathy may occur in those receiving treatment. Periodically monitor blood pressure and adjust or start anti-hypertensive therapy as required.
- Seizures may occur in patients treated with Vadadustat. Following initiation of therapy, monitor patients precisely for premonitory neurologic signs and symptoms. Patients should contact their healthcare practitioner for new-onset seizures, premonitory symptoms, or changes in seizure frequency.
- Vafseo (Vadadustat) tablets should not be used during pregnancy due to potential risks to the fetus. It is also not recommended during breastfeeding. Consult a healthcare provider for safer alternatives or guidance on managing conditions requiring Vafseo treatment while considering pregnancy or breastfeeding.
