Zevtera® (ceftobiprole medocaril) is a broad-spectrum cephalosporin antibiotic used primarily against complex bacterial infections. It is used for the treatment of
- adult patients with Staphylococcus aureus bloodstream infections (bacteremia) (SAB), including those with right-sided infective endocarditis.
- adult patients with acute bacterial skin and skin structure infections (ABSSSI)
- adult and pediatric patients (aged 3 months to less than 18 years) with community-acquired bacterial pneumonia (CABP).
Recommended Dosage: The recommended dosage of Zevtera for adult patients with SAB, ABSSSI and CABP is as follows:
- SAB: The recommended dose is 667 mg administered every 6 hours on Days 1 to 8 then every 8 hours from Day 9
- ABSSSI: The recommended dose is 667 mg administered every 8 hours
- CABP: The recommended dose is 667 mg administered every 8 hours
Duration of therapy in adults is up to 42 days for SAB and 5-14 days for ABSSSI and CABP. Administer each prepared intravenous infusion solution over 2 hours at a concentration of 2.67 mg/mL.
The recommended dosage of Zevtera for pediatric patients (aged 3 months to less than 18 years) with CABP is as follows:
- Pediatric Patients Aged 12 Years to Less than 18 Years Old: The recommended dose is 13.3 mg/kg (up to 667 mg/dose) administered every 8 hours
- Pediatric Patients Aged Greater than or Equal to 3 Months and Less than 12 Years Old: The recommended dose is 20 mg/kg (up to 667 mg/dose) administered every 8 hours.
- Patients with a known allergy to ceftobiprole, cephalosporins, or any beta-lactam antibiotics should avoid using Zevtera injection. Hypersensitivity reactions, including severe anaphylaxis, can occur. Immediate medical attention is necessary if an allergic reaction develops.
- Antibiotic treatment can alter the normal flora of the colon and permit the overgrowth of Clostridium difficile. This may result in severe diarrhea. If CDAD is suspected, appropriate measures, such as discontinuation of the antibiotic, should be taken.
- The prolonged use of antibiotics can lead to the overgrowth of non-susceptible organisms. Continuous monitoring is advised, and if a superinfection occurs, appropriate measures should be implemented.
- Dosage adjustments are required for patients with moderate to severe renal impairment. Regular monitoring of renal function is recommended to avoid potential toxicity.
- Patients should be monitored for signs of hematologic reactions such as neutropenia, thrombocytopenia, or anemia. Regular blood counts are advisable, particularly during prolonged therapy.
- To minimize the development of drug-resistant bacteria, Ceftobiprole Medocaril should be used only to treat infections that are confirmed or strongly suspected to be caused by susceptible bacteria. Prescribing it in the absence of a confirmed bacterial infection increases the risk of resistance.
- Safety and efficacy in pediatric patients have not been established. Pregnant and breastfeeding women should only use Zevtera if clearly needed, and after a thorough evaluation of the potential risks and benefits.
