Austedo is a vesicular monoamine transporter 2 (VMAT2) inhibitor used for the treatment of:
- the involuntary movements (Chorea) related to Huntington’s disease
- movements in the face, tongue, or other body parts that cannot be controlled (Tardive dyskinesia) in adults.
Dosage of Austedo can be titrated at weekly intervals by 6 mg/day on behalf of reduction of chorea or tardive dyskinesia, and tolerability, up to a maximum recommended daily dosage of 48 mg (24 mg twice daily). Administer total daily dosages of 12 mg or more in a couple of divided doses.
For individuals at risk for QT prolongation, evaluate the QT interval prior to and after titrating the total dosage above 24 mg/day.
- Individuals with Huntington’s disease are at higher risk for depression and suicidal behaviors. Austedo tablets may elevate the probability of suicidal thoughts in those with Huntington’s disease. All those who express suicidal ideation must be monitored promptly.
- As a progressive disorder, Huntington’s disease is characterized by cognition, changes in mood, chorea, functional potential, and rigidity over time. VMAT2 inhibitors like Austedo may cause a worsening in mood, rigidity, cognition, and functional potential. Prescribers must frequently re-assess the requirement for Austedo treatment in their patients by evaluating the effect on chorea and probable side effects.
- A potentially critical symptom complex occasionally referred to as Neuroleptic Malignant Syndrome (NMS) may occur in association with medicines that scale down dopaminergic transmission. If Austedo therapy is required following recovery from NMS, patients need to be evaluated for symptoms of recurrence.
- Treatment with Austedo may increase the probability of agitation, akathisia, and restlessness in those with tardive dyskinesia and Huntington’s disease. Patients receiving this medicine should be evaluated for signs and symptoms of agitation and restlessness.
- Deutetrabenazine may be responsible for causing parkinsonism in individuals with Huntington’s disease. If parkinsonism occurs during therapy, the dosage needs to be reduced; some patients may need discontinuation of this therapy.
- Sedation is a dose-limiting side effect of Deutetrabenazine. Patients should avoid performing acts that require mental alertness such as operating a motor vehicle or operating hazardous machinery, until they are on a maintenance dose and know how the medicine affects them.
