Idhifa is an isocitrate dehydrogenase-2 inhibitor used for the treatment of adult patients with relapsed or refractory acute myeloid leukemia (AML) with an isocitrate dehydrogenase-2 (IDH2) mutation.
Recommended Dosage: Select patients for the treatment of acute myeloid leukemia (AML) with Idhifa based on the presence of IDH2 mutations in the blood or bone marrow.
The recommended starting dose is 100 mg taken orally (by mouth) once daily with or without food until the disease is progressive or unacceptable toxicity occurs. For those without disease progression or unacceptable toxicity, treat for a minimum of six months to enable time for clinical response.
It is not advisable to split or crush the tablets. Administer tablets of Idhifa 100 mg by mouth persistently every day.
If a dose is vomited, missed, or not taken at the usual time, administer the dose as quickly as possible on the same day, and return to the normal schedule the following day.
- Evaluate blood counts and blood chemistries for leukocytosis and tumor lysis syndrome (TLS) prior to the start of Enasidenib and monitor at a minimum of every 14 days for at least the first 3 months during treatment. Manage any kind of abnormalities promptly.
- Patients treated with Enasidenib may experience differentiation syndrome, which may be fatal or life-threatening if not treated. Differentiation syndrome is related to rapid proliferation and differentiation of myeloid cells.
- In case of differentiation syndrome, initiate oral or intravenous corticosteroids (e.g., 10 mg dexamethasone every 12 hours) and hemodynamic monitoring until improvement. Hospitalization for close observation and evaluation of patients with pulmonary or renal manifestation is required.
- Enasidenib 100 mg can cause embryo-fetal harm if used by a pregnant woman. Males with female partners of reproductive age should use effective contraception throughout treatment and for at least 30 days after the last dose. Pregnant females, patients becoming pregnant while on Enasidenib, or male patients with pregnant female partners must be apprised of the potential risk to the fetus.
- Women taking treatment with Enasidenib tablets should not breastfeed during treatment and for at least 30 days (1 month) after the final dose of Enasidenib.
