Inrebic is a kinase inhibitor used to treat adult patients with intermediate-2 or high-risk primary or secondary (post-polycythemia vera or post-essential thrombocythemia) myelofibrosis (MF).
Recommended Dosage: The recommended dosage is 400 mg taken by mouth once daily for patients with a baseline platelet count beyond or equal to 50 x 109/L. The inrebic capsules may be taken with or without food. Use with a high fat meal may reduce the incidence of vomiting and nausea.
Modify the dose of inrebic for patients using concomitant strong CYP3A4 inhibitors, and in patients with severe renal impairment (creatinine clearance (CLcr) 15 mL/min to 29 mL/min). If a dose is missed, the next scheduled dose needs to be taken the following day.
- Severe and fatal encephalopathy, including Wernicke’s encephalopathy, may occur. Assess thiamine levels in all patients before starting inrebic 100 mg capsule, periodically during treatment. Do not start treatment in patients with thiamine deficiency; replete thiamine prior to initiation of treatment.
- Therapy with Fedratinib can cause anemia and thrombocytopenia. Obtain a complete blood count (CBC) at baseline, periodically during treatment. For Grade-3 thrombocytopenia with active bleeding or Grade-4 thrombocytopenia, interrupt therapy until resolved to ≤ Grade-2. Restart dose at 100 mg in a day below the last given dose and platelets should be monitored.
- Gastrointestinal toxicities are the common side effects in Fedratinib-treated patients. Consider providing apt prophylactic antiemetic therapy (e.g., 5-HT3 receptor antagonists) during therapy with fedratinib 100 mg capsule. Monitor thiamine levels and replete as required.
- Monitor hepatic function, periodically during therapy. For Grade-3 or higher ALT/AST elevations (higher than 5 × ULN), interrupt Fedratinib dose until resolved to Grade-1 or less or to baseline. Restart dose at 100mg daily below the last given dose. If re-occurrence of a Grade-3 or higher elevation of ALT/AST, interrupt treatment with Fedratinib.
- Monitor amylase and lipase, periodically during treatment. For Grade-3 or higher amylase or lipase elevations, interrupt Fedratinib until resolved to Grade-1 or less or to baseline. Restart dose at 100mg in a day below the final given dose.
